دور المسح الذرى فى تقييم وظائف الكلى فى مرضى نقص الدم للكلى

خلاصـــة

يعتبر قصور الدورة الدموية الكلوية سبب رئيسى يؤدى للفشل الكلوى و ثبت أن التغييرات الأولية فى الوظائف الكلوية قد تحدث فى مرضى تصلب الشرايين الغير كلوية و يسبق بداية حدوث القصور فى الدورة الدموية الكلوية.

لتقييم آثار تصلب الشرايين الغير كلوى على الكليتين، تم فحص 50 مريض مصابون بدرجات متنوعة بتصلب الشرايين بدون وجود تغيرات مخبرية أو أكلينيكية فى قصور الدورة الدموية الكلوية و حدوث ارتفاع فى ضغط الدم الكلوى وقد تم عمل النتائج المخبرية، الموجات فوق الصوتية للشرايين لتحديد موضعية المرض فى الأوعية الدموية الخارجية (شرايين طرفية وسباتية) و تقييم غير متداخل للوظائف الكلوية بواسطة المسح الذرى للكلى لتقييم تدفق الدم الكلوى باستخدام "م ا ج₃" و الرشح الكلوى بواسطة "د ت ب ا" بالإضافة الى نسبة الكولسترول ونسبة الدهون عالية و قليلة الكثافة و الجلسريدات الثلاثية و كذلك المرضى المدخنين بالإضافة الى دمج نتائج الموجات فوق الصوتية على ضيق الشجرة الشريانية ثم تجميع الأماكن طبقاً لدرجة الضرر بتصلب الشرايين الوعائى.

أظهرت النتائج عدم وجود تغير فى كرياتنين البلازما أما الرشح الكلوى بواسطة "د ت ب ا" فزاد من 91.58 ± 26.53 الى 93.47 ± 24.82 مللى/دقيقة/1.73 م2 وهبطت نتائج تقييم التدفق الدموى الكلوى بواسطة "م ا ج₃" بإضراد مع درجة إصابة الأوعية الدموية من 244.86 ± 60.60 الى 173.59 ± 58.74 مللى/دقيقة/1.73 م2. أشار التحليل نكوص متدرج مضاعف فى نتائج التدفق الدموى الكلوى بواسطة "م ا ج₃" أفضل مظاهر ضيق الشجرة الشريانية وكذلك فى المرضى المدخنين و نسبة الكولسترول قليل الكثافة فى الدم.

شكلت النتائج دليل صائب لوظائف الكلى فى المرحلة الأولية الكامنة لمرضى قصور الدورة الدموية الكلوية وتؤكد أن الوظائف الكلوية يجب تقييمها بعناية فى مرضى قصور الدورة الدموية الخارجية خاصة المرضى المعرضين لعوامل الخطر لمرض الشرايين التاجية.

ABSTRACT

Ischemic nephropathy is an important cause of renal failure.  Sub-clinical renal function abnormalities may exist in patients with extra renal atherosclerosis and may precede the onset of overt ischemic nephropathy.

To assess the impact of extrarenal atherosclerosis on the kidney, the study evaluated renal function in 50 subjects with differing degrees of peripheral atherosclerosis without manifest clinical or laboratory signs of ischemic nephropathy and renovascular hypertension.  All laboratory testing including total LDL and HDL-cholesterol, triglycerides, ultrasonography with Doppler analysis for the localization of peripheral vascular disease (carotid and lower limb arteries), and non-invasive evaluation of renal function by radionuclide studies of renal plasma flow (MAG₃ clearance) and glomerular filtration (DTPA clearance) were determined as well as smoking habit was recorded. By combining sonographic data on arterial tree stenosis (ATS), the subjects were grouped according to the atherosclerotic vascular damage (ATS involvement).

The results showed no change in plasma creatinine while DTPA clearance was increased from 91.58±26.53 to 93.47±24.82 ml/min/1.73m². MAG₃ clearance was progressively declined with the severity of vascular damage from 244.86 ± 60.60 to 173.59±58.74ml/min/1.73m². Stepwise, multiple regression analysis indicated that MAG₃ clearance was best explained by ATS involvement (standardized B coefficient -0.40; P< 0.001), smoking habit (-0.34;P=0.004) and serum LDL-cholesterol (-0.24; P<0.035).

It could be concluded that the renal hemodynamic profile in atherosclerotic patients might constitute functional evidence of the silent phase of ischemic renal disease. The findings suggest that renal function should be carefully assessed in patients with extrarenal atherosclerosis, particularly in those with classic cardiovascular risk factors.
